So, everyone keeps talking about creating an NDIS plan, but really what is it? Essentially, it’s a plan that informs us about your needs and goals and outlines the services and support necessary to meet your requirements. An NDIS plan enables you to get funding for all of this and takes the pressure off. So ultimately, we can provide…care your way.
Planning your plan
To access funding, you’ll meet with an NDIS consultant and the finalised plan that’s agreed upon by both you and your consultant will be implemented for 12 months. LHS support coordinates will help organise assessments and provide materials and resources utilised in the meetings, such as personal information and current services. Anything you need, we’ve got you.
Bring your plan to action
The next step is to bring your plan to action and to managing it.
Your plan can be managed directly by you or nominee (mum or dad), a registered plan management provider or the NDIA. If wish to have a plan management provider, we can function as the middlemen by assist with financial intermediary and more. Our plan management team will work tirelessly to manage your fund and coordinate your support needs. We can help you decide on what type of support you need, who the best provider is and anything else. We can also create service agreements and make necessary arrangements on your behalf.
We provide support and services
Once you’ve determined, through the help of a support coordinator, what type of support is required, the providers, logistics and costs – we deliver the services and support we promised.

Based on your individualised plan, we pinpoint what you need and provide exceptional support so that your goals are achieved and our impact leaves a meaningful impression. We also provide you and NDIS with a report which outlines your goals, the outcomes, the successes and how we can improve to make your experience better. Review your plan
After 12 months your plan will be reviewed with your support coordinator. If circumstances arise that require your plan to be reviewed sooner, we will make appropriate arrangements.
You can reflect on the services that best helped you achieve your goals. You can revise and modify the plan based on your current and updated needs. So, when your 12-month review comes around with the NDIS, LHS will be there to help you through the journey.
